This course is part of the RCOG Core Knowledge series.

An ectopic pregnancy is a pregnancy which implants at a site other than the endometrium of the uterine cavity. They are most commonly sited in the fallopian tube, however can occur more rarely in the cervix, Caesarean section scar, interstitium, ovary or abdominal cavity.

Ectopic pregnancies are common with approximately 12,000 diagnosed each year in the UK. Despite advances in diagnosis and treatment, unexpected tubal rupture with massive haemorrhage still remains a significant source of maternal morbidity and mortality worldwide.

They remain the leading cause of maternal death in the first trimester of pregnancy and although mortality rates are low (0.2/1000), women continue to die in the UK from ectopic pregnancies.

It is therefore vital that healthcare professionals have a high index of suspicion for this diagnosis and a low threshold to refer to appropriate services, if suspected. Ensuring early referral and easy access to specialised early pregnancy care is crucial in providing an early and safe diagnosis, and reducing morbidity and mortality.

When you have completed this course, you will be able to:

  • Perform an appropriate and safe assessment of patients presenting with symptoms suggestive of ectopic pregnancy
  • understand the application and limitations of ultrasound scanning and quantitative ß-hCG testing
  • formulate a safe management plan for pregnancies of unknown location (PUL) and tubal ectopic pregnancies
  • arrange appropriate follow up for patients with tubal ectopic pregnancies
  • have a basic understanding of the management of non-tubal ectopic pregnancies
  • provide advice regarding future pregnancies.
