Welcome to this Core Knowledge course on Early pregnancy loss - trophoblast diseases.
Gestational trophoblastic disease (GTD) consists of a range of pregnancy-related disorders originating from disordered placental proliferation.
Premalignant disorders include complete and partial hydatidiform mole. Malignant disorders are termed Gestational Trophoblastic Neoplasia (GTN) and include invasive mole, choriocarcinoma, rare placental site trophoblastic tumour (PSTT) and epitheloid trophoblastic tumour (ETT).
GTD is an uncommon occurrence in the UK with a calculated incidence of one in 714 live births. There is evidence of ethnic variation with women from Asia having a higher incidence. GTD is also associated with extremes of age at conception (1/500 in women < 15yrs , 1/8 in women >50yrs). It can develop after a molar pregnancy, non molar pregnancy or live birth. The incidence after a live birth is estimated at 1: 50000.
Despite its relative rarity, GTD remains an important disorder for clinicians to recognise because it is nearly always curable (cure rate is 98-100%) with retention of fertility. This can be attributed to improved management protocols, development of effective treatment, use of human chorionic gonadotrophin as a biomarker and centralisation of care to specialist centres.
When you have completed this course, you will be able to:
- understand and demonstrate a comprehensive knowledge of the terminology, epidemiology, genetics, clinical presentation and ultrasound diagnosis of GTD
- identify the differences in histopathology and genetics in relation to GTD
- understand the preference for treatment by surgical uterine evacuation
- recognise the need for registration of GTD with trophoblastic screening centre (assuming UK-based practice) and organise appropriate referral
- exhibit an understanding of the indications for chemotherapy for GTD
- classify gestational trophoblastic neoplasia using FIGO prognostic and anatomical staging systems and demonstrate knowledge of low- and high-risk classification
- demonstrate a comprehensive understanding of choice for contraception and have the desired skills to offer advice regarding future pregnancies in women with previous molar disease and how this varies if chemotherapy was required.
