The NOTSS forms can be completed in whatever style you feel most comfortable, for example, you could use:
- the passive tense
- the first ("I"), second ("you") or third ("he/she") person
- full sentences or short bullet-pointed notes.
As previously mentioned, you could focus on one particular area of, for example, situational awareness, rather than writing down everything that happened during the assessment period. You could also choose to do a NOTSS assessment that solely focuses on one area (e.g. communication). You should use the NOTSS form in whatever way you find most useful.
The folllowing pages give some examples of NOTSS feedback for each section on the form. Please note that these examples are not exhaustive, and that the nature of the feedback will depend on the level of the trainee, the situation and the focus of the assessment.
